How Many Shockwave Therapy Sessions Do You Need? An Honest Answer
The Honest Answer: It Depends — But Here's the Real Framework
This is one of the most common questions Dr. Anthony Consales hears from new patients at Consales Chiropractic in White Oak, PA. And it's a completely fair one to ask before committing to any treatment.
The truthful answer isn't a single number. The number of shockwave therapy sessions you'll need depends on three factors: the specific condition being treated, how long you've had it, and how your body responds to treatment. But it's not random — there are clear patterns that help us set realistic expectations from the very first visit.
Here's what 35+ years of clinical experience and the published research actually tell us.
The General Range: 3 to 6 Sessions for Most Conditions
For the majority of chronic musculoskeletal conditions — tendonitis, plantar fasciitis, knee pain, shoulder pain — most patients complete a course of 3 to 6 shockwave therapy sessions spaced roughly one week apart.
That's the honest baseline. Some patients need fewer. Some need more. Here's why.
How Many Sessions by Condition
Plantar Fasciitis
One of the best-researched applications for shockwave therapy. Most patients with plantar fasciitis respond within 3–5 sessions, particularly those who haven't had repeated cortisone injections (which can damage fascia tissue and complicate healing). Patients who've had plantar fasciitis for less than 12 months tend to respond faster than those with multi-year histories.
Achilles Tendinopathy
Typically 3–5 sessions. The Achilles responds well to StemWave acoustic wave therapy because it's a vascular-poor tendon — meaning it doesn't get much natural blood supply for healing. Shockwave's ability to stimulate neovascularization (new blood vessel formation) directly addresses that root problem.
Knee Pain & Patellar Tendinopathy
For knee conditions treated with StemWave, 3–6 sessions is typical. When combined with Knee On Trac™ mechanical traction, many patients with chronic knee pain — including arthritis — see meaningful improvement within the first few visits. Our $49 introductory session for StemWave lets you experience the effect before committing.
Rotator Cuff & Shoulder Tendinopathy
3–6 sessions for most patients. Calcific shoulder tendinopathy (where calcium deposits have formed in the tendon) often requires the higher end of that range, but many patients see significant softening of calcifications and pain reduction within 4 sessions.
Tennis Elbow / Golfer's Elbow (Lateral/Medial Epicondylitis)
Typically 3–5 sessions. Elbow tendons respond well to shockwave, especially when the underlying biomechanical load (grip patterns, wrist position) is also addressed.
Neuropathy
Peripheral neuropathy — including diabetic neuropathy — often requires a longer course, sometimes 6–10+ sessions, because nerve tissue regenerates more slowly than tendon or muscle. Results are cumulative and continue to improve for weeks after treatment ends.
Hip Pain
Gluteal tendinopathy and hip bursitis typically respond within 4–6 sessions. Our post on hip pain treatment in Western PA covers this in more detail.
Why Some Patients Feel Results After the First Session
It's not uncommon — and it's not a placebo. Here's the science behind early response:
StemWave's electrohydraulic acoustic waves immediately stimulate pain-inhibiting nerve fibers (the gate-control mechanism) and begin triggering the release of substance P — a neuropeptide involved in pain transmission. Within the first 24–48 hours, local blood flow increases and cellular repair signals are activated.
Many patients leave their first StemWave session at our White Oak clinic feeling noticeably lighter or less guarded in the treated area. Some experience significant relief after visits 1–3. Others don't feel a dramatic shift until around session 4 or 5 — and then improvement accelerates rapidly.
This is why we offer a $49 introductory StemWave trial: so you can feel what your body's response is before making any long-term commitment. Results in the first session are genuinely informative.
What Makes Treatment Take Longer
A few factors can push the number of sessions higher:
- Duration of the condition — a 5-year chronic tendinopathy has more disorganized tissue and more ingrained compensation patterns than a 6-month problem. More sessions are typically needed.
- History of cortisone injections — cortisone can thin and weaken tendon tissue over time, making repair more complex
- Calcific deposits — calcifications require higher energy delivery and may need additional sessions to fully resolve
- Nutritional deficiencies — the body cannot repair tissue without proper raw materials. This is why Dr. Consales pairs StemWave with Nutrition Response Testing for patients who aren't responding as expected. NRT identifies whether your body has the vitamins, minerals, and cofactors it needs to actually complete the repair cycle. See how these therapies combine into a whole-body healing system.
- Ongoing mechanical stress — if you're continuing an activity that re-injures the tendon between sessions, progress will be slower
How Often Are Sessions Scheduled?
Typically once per week. This spacing allows the biological cascade triggered by each session — new blood vessel formation, collagen synthesis, stem cell activation — to unfold before the next treatment reinforces and extends the process.
In some cases, especially early in treatment when pain is severe, Dr. Consales may recommend sessions every 5–7 days. Once the tissue is responding well, sessions can often be spaced further apart as maintenance.
What About StemWave vs. Older Shockwave Devices?
Not all shockwave therapy is the same. Many clinics use older pneumatic (air-pressure) devices, which generate a lower-energy, less penetrating wave. These may require more sessions — sometimes 6–10 — to achieve results that StemWave's electrohydraulic technology can often produce in 3–5.
StemWave uses true electrohydraulic acoustic wave generation — the same physics used in high-end research settings. The wave penetrates deeper, energizes tissue more broadly, and tends to produce faster responses. Read more about how StemWave differs from traditional shockwave.
The Bottom Line: What to Expect at Consales Chiropractic
When you come in for a StemWave evaluation, Dr. Consales doesn't hand you a preset package and send you to the front desk. He assesses your specific condition, your history, your tissue response, and your goals — and gives you a realistic treatment recommendation based on all of it.
Most patients with standard tendon conditions (plantar fasciitis, rotator cuff, Achilles, tennis elbow, knee) complete treatment in 3–6 weekly sessions. Many feel meaningful improvement by session 2 or 3. A small number of complex or long-standing cases need 8–10. We'll tell you which category you're likely in from the start — and we'll reassess as we go.
Our office is at 3045 Jacks Run Rd, White Oak, PA 15131, serving patients from North Huntingdon, McKeesport, Irwin, Greensburg, and throughout Western PA.
