Consales Chiropractic is committed to protecting the privacy of your protected health information (PHI). We are required by the Health Insurance Portability and Accountability Act (HIPAA) to maintain the privacy of your PHI, provide you with this notice, and follow the terms currently in effect.

  • Healthcare Operations: To support business activities such as quality review, staff training, and practice management.
  • Appointment Reminders: To contact you about appointments or treatment recommendations.
  • As Required by Law: When required by federal, state, or local law.

Uses That Require Your Authorization

Most uses and disclosures of psychotherapy notes, marketing communications, and sale of PHI require your written authorization. You may revoke authorization at any time in writing.

  • Request an amendment to your records
  • Request an accounting of certain disclosures
  • Request restrictions on certain uses and disclosures
  • Request confidential communications (e.g., by phone vs. mail)
  • Receive a paper copy of this notice upon request
  • Be notified of a breach of unsecured PHI

Information submitted through our website contact forms is used only to respond to your inquiry and schedule care. Please do not include sensitive medical details in online forms — share those during your in-office visit or by phone.

Complaints

If you believe your privacy rights have been violated, you may file a complaint with our office or with the U.S. Department of Health and Human Services, Office for Civil Rights. You will not be retaliated against for filing a complaint.
