What Is Vertigo, Really?

When these crystals move around, they disrupt the signals your inner ear sends to your brain about balance and spatial orientation.

The result? Short but intense bursts of spinning that are triggered by specific head movements — rolling over in bed, looking up, bending forward, or turning your head quickly. BPPV can develop after a head injury, an inner ear infection, or sometimes for no obvious reason at all.

Cervicogenic Vertigo

Cervicogenic vertigo originates not in the inner ear, but in the cervical spine — specifically the upper portion of your neck. This is the type that often gets missed or misdiagnosed, and it's where chiropractic care can make a dramatic difference.

Your upper cervical spine (the top two vertebrae, called the atlas and axis) is home to a dense network of sensory receptors that constantly communicate with your brain about body position and movement. When these vertebrae are misaligned, they can interfere with those signals — essentially sending your brain conflicting information about where your body is in space. The result can feel identical to inner ear vertigo, but the root cause is mechanical, not neurological.

The Cervical Spine — Balance Connection You Probably Haven't Heard About

Most people don't realize just how intimately connected the neck and the balance system are. Here's why this matters:

  • Blood flow to the inner ear: The vertebral arteries run directly through the cervical vertebrae on their way to the brain and inner ear. Misalignment in the upper cervical spine can restrict or alter blood flow through these arteries, affecting the function of the vestibular system — your body's internal balance control center.
  • Proprioceptive interference: The joints and muscles of the upper cervical spine are packed with proprioceptors — sensors that feed constant position data to your brain. Misalignment disrupts this feedback loop, causing your brain to receive mixed signals from your neck, inner ear, and eyes simultaneously.
  • Nerve compression: Structural problems in the cervical spine can irritate or compress nerves involved in vestibular processing, contributing to chronic dizziness, brain fog, and balance issues.

  • BPPV: Many patients see significant improvement after just 1–3 sessions using the Epley maneuver. For straightforward cases, resolution can be rapid and lasting.
  • Cervicogenic vertigo: Most patients begin to notice real improvement within 4–8 visits of upper cervical care. Chronic cases may require a longer course of care to achieve lasting stability.
  • Mixed or complex cases: If your vertigo involves both spinal and inner ear components, or has been present for years, expect a more gradual recovery — but patients consistently tell us that steady progress is motivating, even when complete resolution takes time.
